***REQUIREMENTS FOR ON-LINE SUBMISSIONS***

  • You must be listed as a primary contact, secondary contact, coach or assistant coach on the team locator page.

  • If you are an NCHE member, being listed on the team locator page will allow you access thru the admin button on the tool bar.

  • If you are not an NCHE member and are a primary contact, secondary contact, coach or assistant coach on the team locator page, you must request an invitation to the webpage from the NCHE office.

Registration Form

1) Each Program should use it's county or city designation (e.g.  Wake or Raleigh) when registering rather than a mascot name (e.g. Eagles).

2) Use a separate form for each sport (e.g. baseball, basketball).  Boys and girls teams and different age divisions within the same sport may go on the same form (e.g. Middle School Boys, Varsity Girls).  But, boys and girls soccer would go on different forms since they play in different seasons.

3) If the form is submitted on-line or by e-mail, the submission constitutes a signature.  If it is mailed, it must be signed and typed out.  No hand written forms are accepted.

 

Roster Form

1) Each Program should use it's county or city designation (e.g.  Wake or Raleigh) when registering rather than a mascot name (e.g. Eagles).

2) Be sure to fill in all columns on each player (name, height, number).

3) Last name must  be spelled as it is on the ORANGE card from DNPE.

4) School name  must  be spelled as it is on the DNPE ORANGE card.

5) Chief Administrator name must  be the same name as appeared on the original Notice of Intent (NOI) submitted to DNPE.  If the father's name was originally submitted, do not enter the spouses name now.

6) If the school is not found after doing the above steps, then be sure that the family mailing address was updated with DNPE following any change of address since the NOI was submitted.

Junior/Senior Declaration Page

1) This form may only be filled out by the CUSTODIAL PARENT(S).

2) It must be signed by the Custodial Parent(s) and submitted by mail.

3) "Homeschool Program"  refers to the name of the homeschool team for whom the player will be competing.

4) Since the declaration of year in school is binding, except for medical or family hardship appeal approved by NCHEAC, it should not be filled out unless the athlete is expected to graduate within 2 school years if a junior or 1 school year if a senior.
